Office Closing Update 03/27/23

•  We are making every effort to guide each individual patient on next steps. In many cases your primary care provider will be able to resume care of your endocrine condition.  It is proving difficult for some of our patients to get in with an endocrinologist in the area in a timely fashion as many practices are booked until early 2024.

•  If you are unsure about whether you need to see a new endocrinologist or if you could be managed by your primary provider please contact the office, I will review your records and we will advise you.

•  If you do need to see a new endocrinologist, when you are contacting new endocrinology practices to make an appointment be sure to confirm that the practice is taking new patients AND that you will be able to be seen there before requesting that your records be sent.

•  Please look at the number of refills remaining on medications we have been prescribing you and contact us ASAP if you need refills to bridge you to your appointment with your new endocrinologist OR make sure you have a provider that is willing to fill these prescriptions between when our practice closes and when you have your 1st appointment with your new provider

• Contact my office no later than June 1,2023 if you would like to obtain or transfer your medical records. Click here to access the Release of Information form.
Please print, sign and either mail, fax or email it to us.

It has been a true pleasure to be involved in the care and lives of so many people.  Your continued health is important to me and I want to make sure these next few months are as stress-free and seamless as possible.
